The update section of the App store shows the update has applied & I have tried to download server once more from the store & it still only downloads 4.1. It is a free update, 5.0.3 is supposed to be compatible with OS X 10.10.5 & 10.11.

After running system update we are back up and running but this happened earlier this week as well. Sounds like something is wrong with your upgrades. Our Profile server has been upgrading on it's own for quite some time.
It's getting upgraded to 10.11 this weekend though. No, that’s normal behavior for Server.app. Often, when you upgrade, you’ll have to run through the configuration assistant again, and it just verifies things like your domain and admin email address. However, until you do so, the services will be stopped.
